Abstract:

This is a report on Victoria's HACC Minimum Data Set (MDS) for the 12 month period from 1st July 2003 to 30th June 2004. The report illustrates that the program assists a wide range of people across all age groups. 66% of people assisted are aged 70 or older, but there are also significant number of adults in their middle years, who require assistance with activities of daily living because of various disabling conditions. A useful report for HACC clients, providers and planners in making services more responsive and successful.
