Aged care in Australia, as throughout most developed countries, has undergone substantial change in recent years. These changes have come about in response to a number of concerns, including the sizable imbalance between residential and community care services; the need to expand community care services and improve planning, assessment and targeting; and inadequate intergration and coordination at both service provider and policy levels. Ther has been particluar concern regarding provision of community care for very vulnerable individuals at the margin of entry to residential care. This chapter compares two state approaches (Victoria and Western Australia) to case management in community care, providing a useful perspective on the impact and effectiveness of different appoaches to case management in community care programs.
