Abstract:

There are many ways to organize and classify human errors. They can be divided into 2 categories: intentional and unintentional. They can be classified in terms of their cost - a recovered error has the potential to cause damage but due to luck or good human factors design nothing bad happens. An unrecovered error is one where it is not possible to prevent serious consequences.
