The Joint Standing Committee on the National Disability Insurance Scheme conducted an inquiry into the NDIS Quality and Safeguards Commission about the tragic death of Ms Ann-Marie Smith. The resulting report makes 30 recommendations, to improve the role of the NDIS Quality and Safeguards Commission and ensure the safety of the participants.
