FACS Tenancy Team mailbox for NSW OT reports

FACS Tenancy mailbox is a new resource that will assist external OTs have disability modifications actions addressed in NSW. OTs should cc their Reports to the relevant NSW FACS Tenancy Team mailbox.

Please note that disability modifications undertaken for a FACS tenant residing in a LAHC-owned property are subject to the Disability Modifications policy.

The Occupational Therapist Summary Report is a template with four columns, and these must be completed and accompany the Report prepared by the OT.

Columns shown in blue labelled ‘Identified Disability/Barrier’ and ‘Requested related Home Modification’ assists LAHC (as the property owner) evaluate the suitability of the property.
