The ‘A Way to Stay Home Modification Assessment and Planning Tool’ has been designed to help you through the planning, assessment and construction quoting stages of home modification installations. It can be for your own personal use as an ‘idea starter’; or it can be used in conjunction with the support of an Occupational Therapist (OT) or an OT and Builder team to seek an appropriate plan.
This planning tool has been developed by SCOPE Access. It is available as a download below and online here.
